목적 : 본 연구의 목적은 성인 입원환자의 낙상위험을 사정하기 위해 개발된 FAS-K를 MFS 및 JHFRAT와 비교하여 적극적인 낙상관리가 필요한 중재시기를 결정하기 위해 FAS-K의 경계 기준점(cut-off point)을 확인하는 것이다.
방법 : 본 연구는 FAS-K의 낙상 고위험군 판단 점수를 찾기 위한 방법론적 연구로서, 연구대상자는 2018년 11월 1일부터 2019년 2월 28일까지 서울 소재 2개 상급 종합병원 내, 외과병동에 입원한 성인환자를 대상으로 하였다. 연구조사자는 일주일에 2회, 오전 10시에서 12시 사이에 한 대상자에게 FAS-K, MFS, JHFRAT 세 도구를 적용하여 낙상위험 사정점수를 측정하였다.
결과 : FAS-K와 MFS, JHFRAT 간의 상관분석 결과, FAS-K와 MFS(r=.70, p<.001) 및 JHFRAT(r=.82, p<.001) 모두 통계적으로 유의한 양의 상관관계를 나타냈다. FAS-K의 ROC 곡선 분석결과, FAS-K 값이 4점인 경우 민감도 85.3%, 특이도 49.4%, 양성예측도 8.5%, 음성예측도 98.4%로 나타나 FAS-K의 낙상 고위험군 판단의 경계점수는 4점으로 확인되었다.
결론 : 본 연구의 결과를 토대로 국내병원 성인 입원환자를 대상으로 낙상 고위험군 판별하기 위해 FAS-K를 우선적으로 사용할 수 있을 것이다. 또한 FAS-K 4점을 낙상 고위험군으로 분류하여 적극적인 낙상예방 중재를 제공할 시점으로 활용할 수 있을 것이다.

Purpose: The purpose of this study was to test the predictive validity of the Fall Assessment Scale-Korean version (FAS-K) and to find the most appropriate cutoff score to screen high-risk fall groups in adult patients in general hospitals in Korea.
Methods: We performed a prospective evaluation study in medical and surgical ward patients at two major general hospitals in Seoul. Data were collected from Nov. 1, 2018 to Feb. 28, 2019, nurses performed 651 observation series. The researcher measured the fall risk assessment score by applying FAS-K, MFS (Morse Fall Scale), and JHFRAT (Johns Hopkins Hospital Fall Risk Assessment tool) to the patients twice a week between 10 am and 12 noon. Data were analyzed using Pearson's corelation coefficients, and the sensitivity, specificity, predictive value, and the area under the curve (AUC) of the three tools.
Results: The FAS-K was positively correlated with the MFS (r=.70, p<.001) and the JHFRAT (r=.82, p<.001). According to the receiver operating characteristics (ROC) curve analysis of the FAS-K, sensitivity, specificity, and positive and negative prediction values were 85.3%, 49.4%, 8.5%, and 98.4%, respectively, when the FAS-K score was 4. Therefore, the cut-off score of the FAS-K to identify groups with high fall risk was 4.
Conclusion: The FAS-K is a valid tool for measuring fall risk in adult inpatients. In addition, the FAS-K score, 4, can be used to identify high-risk fall groups and know specific points in time to provide active interventions to prevent falls.
